* Removed all of the hard-wraps
* Added leading hyphens when missing
* Added related mr.xxx or issue.xxx entries
* Added Fix: Change: Add: keywords when missing
When "support" was added, marked with "Change:" keyword
* Added missing ":" for "Introduce" lines
* Removed the duplicated adds in the "-Add: adds...." constructs
* Merged 4 "returns xrt_result" fragments into one.

xrt: Bump overlay limit from 16 to 128

Based on my tests, there's virtually no difference in CPU and GPU
performance even when increasing RENDER_MAX_LAYERS to 1024.
For now, a value of 128 should be sufficient for everyone.
