Frederic Plourde 55f8bbe13c doc: More cleaning up of changelogs 
* Removed all of the hard-wraps
 * Added leading hyphens when missing
 * Added related mr.xxx or issue.xxx entries
 * Added Fix: Change: Add: keywords when missing
    When "support" was added, marked with "Change:" keyword
 * Added missing ":" for "Introduce" lines
 * Removed the duplicated adds in the "-Add: adds...." constructs
 * Merged 4 "returns xrt_result" fragments into one.

knuxify 82e59dcce6 d/daydream: Initialize mutex and thread 
Without this, when a Daydream controller was paired, the monado service would
crash at startup with the following message:

  Assertion failed: oth->initialized (src/xrt/auxiliary/os/os_threading.h: os_thread_helper_start: 509)

Korcan Hussein f7ea9c575f st/oxr: use correct verify error codes for gfx bindings check 
CTS expects any/all invalid graphics bindings for xrCreateSession calls to return
XR_ERROR_GRAPHICS_DEVICE_INVALID rather than XR_ERROR_INITIALIZATION_FAILED.

Korcan Hussein f24ee92626 st/oxr: disable stencil-only swapchain formats (VK_FORMAT_S8_UINT) 
Stencil-only formats cause VK_ERROR_DEVICE_LOST on NVIDIA GPUs when
using the compute based pipeline. The failure occurs
during image layout barrier transitions in swapchain
initialization.

Since no OpenXR core or extensions currently require stencil-only
swapchains, disable them until a (driver) fix is available.

Korcan Hussein 69ea033996 st/oxr: Fix hand joint location flags not cleared when inactive 
Consolidate error/inactive paths in oxr_session_hand_joints() to
ensure joint location and velocity flags are always properly cleared
when hand-tracking is not active.
