Frederic Plourde 55f8bbe13c doc: More cleaning up of changelogs 
* Removed all of the hard-wraps
 * Added leading hyphens when missing
 * Added related mr.xxx or issue.xxx entries
 * Added Fix: Change: Add: keywords when missing
    When "support" was added, marked with "Change:" keyword
 * Added missing ":" for "Introduce" lines
 * Removed the duplicated adds in the "-Add: adds...." constructs
 * Merged 4 "returns xrt_result" fragments into one.
